You can only register as a new patient when you’re living on the Zuidas.

We can sometimes make an exception for family members of patients who are already registered (as long as living within our service area), you can contact us by email.

You can register by filling in the form via the WebApp.

We will review your registration within 3 working days, you will receive a confirmation email.
